By accessing or using contour.health (the "Site"), operated by Contour Performance MSO LLC and Contour Performance PLLC ("Contour," "we," "us," or "our"), you agree to these Terms & Conditions. If you do not agree, please do not use the Site.
2. About Our Services
Contour Health is a membership-based clinical practice offering Core and Core MD membership tiers, along with a separate GLP-1 weight management program, delivered via telehealth and, where available, in person. Use of the Site to request an appointment or submit information does not by itself establish a patient-provider relationship; that relationship begins only once you have completed our intake process and been accepted as a patient.
3. Not for Medical Emergencies
This Site is not monitored for emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. Do not use this Site, text messaging, email, or our contact forms to communicate urgent or emergency medical concerns.
4. Eligibility
Our services are intended for individuals 5 years of age or older who are located in a state where Contour is licensed to provide care. For patients between the ages of 5 and 18, registration and account setup must be completed by a parent or legal guardian.
5. Text Messaging Terms
By submitting your phone number and opting in, you agree to receive text messages from Contour Health related to scheduling, appointment reminders, and account notifications, as described in our Privacy Policy. Message and data rates may apply; message frequency varies. Reply STOP to opt out or HELP for assistance.
6. Membership & Appointment Policies
Membership tiers, pricing, billing cadence, and cancellation policies are described in our patient-facing membership guide, provided separately during onboarding and available upon request.
7. Intellectual Property
All content on this Site — including text, graphics, logos, and the Contour Health name and branding — is owned by Contour or its licensors and may not be copied, reproduced, or used without our prior written permission.
8. Acceptable Use
You agree not to use the Site to submit false or misleading information, to interfere with the Site's normal operation, or to attempt unauthorized access to our systems.
9. Third-Party Links and Services
Our Site may link to or integrate with third-party services (for example, our scheduling platform). We are not responsible for the content, privacy practices, or terms of those third-party services.
